Production Reports
Per 6 NYCRR 551.2, well operators are required to compile and submit annual production data to DEC no later than March 31st of the following calendar year. Operators are required to submit production data to DEC using either the Annual Well Report or the New York eForm web application.
Annual Well Report
Operators must submit complete and approvable production data to DEC via the Annual Well Report (PDF) (1.6 MB), so that it is received by DEC no later than March 31st. Operators are required to mail the original Annual Well Report to DEC; Email and fax submittals are not acceptable.

New York eForm
DEC encourages operators to submit production data electronically via the New York eForm (NYeForm) web application. Operators must submit complete and approvable production data to DEC via NYeForm no later than March 31st.
To register for NYeForm, an operator must submit a complete and approvable Registration to File Electronic Submissions (PDF) (90 KB) form to DEC. Additionally, all potential NYeForm users must have a valid my.NY.gov ID registered with the same email address provided on the Registration to File Electronic Submission form. To complete the registration process, the operator must contact DEC at [email protected].
